How to calculate a RVSP.

Step 1: Obtain a CWD of the long axis view of the tricuspid valve.Tricuspid regurgitation must be present to make this calculation. Tricuspid regurgitation is present in most normal individuals so this calculation should be able to be made in almost everyone. A CWD of the TR jet will yield a peak velocity. Four times the square of the peak velocity of the TR jet yields the gradient across the valve during ventricular systole. The gradient added to the CVP will yield the RVSP or PASP (in no RVOT outlet obstruction is present).

Step 2: Obtain a CWD of the pulmonary valve. If a gradient does not exist across the pulmonary valve, then the RVSP = PASP. Otherwise, add the gradient to the RVSP to obtain the PASP.
